TI Evaluation of the HIV antibody detection in oral fluid from high risk
subjects in Thailand.

AB OBJECTIVES: To determine the feasibility of using oral fluid for the
detection of HIV antibodies. METHOD: Matched oral fluid and serum
samples were obtained from a hospital for drug user in Thailand. The
Orasure device (Epitope Inc.) was used to collected oral specimens. Sets
of oral fluid and serum were tested with the Vironostika HIV-1
Microelisa (Organon Teknika). Repeated reactive samples and those giving
discordant results were further confirmed by serum Western blotting
(Epitope Inc.) RESULTS: The sensitivity and specificity of oral sample
testing, using the results of serum testing as a gold standard, are 100%
(84/84) and 99.5% (219/220). for HIV antibody detection. Six serum
samples produced inconclusive results by western blot. Four of matched
oral fluid specimens were positive and 2 were negative by EIA.
DISCUSSION AND CONCLUSIONS: HIV antibody testing in oral fluid obtained
by Orasure device is highly sensitive and specific. The use of an oral
sample provide a safer and more convenient procedure than blood sample
for HIV antibody detection with the application in surveillance as well
as those problems on blood collection. However, retesting of blood
sample is necessary for diagnosis purpose. Detection of low level HIV
antibody in oral fluid need some modification of EIA system to get a
good sensitivity and specificity. Disadvantage of this technique is a
high cost of collecting device for oral fluid.
